OUTPATIENT DIABETES EDUCATION PROGRAM

Description

Comprehensive diabetes education program including individual visits, classes and support groups. Topics include treatment options, nutritional management and meal planning, benefits of exercise, medication information, blood glucose monitoring and control, preventing and treating complications of diabetes and setting goals to promote health.Classes are presented by Certified Diabetes Educators, registered nurses and a registered dietician.Our goal is to provide quality education to persons with diabetes and their families.Individual appointments are also available and suggested for those who are unable to attend the class series; to learn self blood glucose monitoring for home testing; to develop and individual meal plan; to discuss special concerns related to pregnancy, insulin pump therapy,children and adolescents and aging.

8:00A.M. - 4:00 P.M. Visits by appointment only. Evenings and early sessions available as needed.
