CASE MANAGEMENT FOR SENIORS - JFS

Description

Provides professional case management services to the elderly. Supplements the agency's Home Companion program. The Case Manager assesses and then makes recommendations to the senior and his/her family that will allow the client to live as independently as possible.Services include assistance with all kinds of daily tasks that a senior may no longer be able to or desire to do. Examples include bill paying; arranging and talking with the doctor; serving as a liaison to the family on behalf of the senior; helping with alternative arrangements, etc. The case manager coordinate necessary services and continues to monitor the client for as long as deemed necessary.

MON. - THURSDAY, 9:00 A.M. - 5:00 P.M. FRIDAY,. 9:00 A.M. - 4:00 P.M.
