CT STATEWIDE RESPITE CARE PROGRAM

Description

The Connecticut Statewide Respite Care Program is funded by the State Department of Social Services (DSS) and is operated in partnership with the Alzheimer's Association Chapters of Connecticut and the Connecticut Area Agencies on Aging. The program offers caregivers the opportunity to receive an assessment of services needed and have a care plan developed and /or purchase services for the individual with dementia. Eligible families may apply for daytime or overnight respite care services including: Adult day care, home health aide, homemaker/companion, skilled nursing care or short term nursing care. The applicant or authorized agent must provide a Physician's Statement declaring that the patient has Alzheimer's Disease or related dementias resulting from: Multi-infarct dementia, Parkinson's Disease, Lewy Body Dementia, Huntington's Disease, Normal Pressure Hydrocephalus, or Pick's Disease. The applicant cannot be covered by Medicaid (Title 19) or by the CT Home Care Program for Elders. They must have an income of no more than $30,000 a year and liquid assets of $80,000 or less.
