Coordinates services for children under age 21 who have or who are at elevated risk for having chronic physical, developmental, behavioral or emotional conditions (biological or acquired), and who also require health and related services of a type or amount not usually required by children of the same age. Transitional services are provided through workshops, as well as family support services and emergency preparedness. Our center covers the towns from Greenwich to Monroe, Stratford and Bridgeport. We do the care coordination and can refer for Respite funding and Extended Service Funds (Title V) to the CT Lifespan Respite Coalition - Joy Liebeskind - our coordinator for these services.
