SYSTEM OF CARE

Description

The System of Care is a collaborative designed to support and strengthen parents and guardians in their efforts to meet the multiple needs of children with serious emotional problems. It provides individualized and comprehensive care to families in their home communities and coordinates with many providers to ensure implementation of services and foster optimal outcomes. System of Care provides services to families in Norwalk, Westport, Wilton, and Weston. Referrals are from the school system, doctor's offices, family members, the community and agencies. Every effort is made to expedite referrals in a timely manner. The Care Coordinator works with the famiy in identifying a child specific team made up of service providers and any other key players in the child and family's life. The family and Care Coordinator decide on an effective treatment plan. The Care coordinator coordinates meetings and communicates with service providers.


The System of Care Community Resource Committee (CRC) meets at 9:00 am. On the 1st Tuesday of each month at the Human Services Council, One Park Street, Norwalk.
