Description

The Norwalk Community Health Center is a federally qualified health center offering primary medical care to all Norwalk residents, regardless of their ability to pay. Mental health services and case management are offered on-site, and dental services are offered by arrangement. Transportation to and from the health center is free to qualified patients and families. NCHC is in its 12th year of operation and in April 2010 relocated to a state of the art facility. In addition to medical care, other programs offered include:

Assistance in determining insurance benefits (HUSKY, Medicaid and sliding scale fee) Ext. 1201 & 1204

Social Services and Behavioral Health: 2 Community Outreach Workers and 2 Clinical Social Workers, including one Spanish speaking clinician. Social Services Educational and Support Groups. Ext.1202 & 1207

Comenzando Bien prenatal health education program for prenatal patients of NCHC. Ext. 1203 & 1208

Child FIRST Program: Intensive clinical/mental health home visiting program for children birth to age 6 and their families, co-located at NCHC, with services provided by the Child Guidance Center of Mid-Fairfield County, in collaboration with Family & Children’s Agency. 203-851-1018

Dental services: A collaboration with West Avenue Dental Associates

Reach Out & Read: Early pediatric reading readiness program for NCHC patients ages 6 months to 5 years

Ryan White & HIV Services

SNAP – representative once a week to expedite Food Stamp application

DSS Mobile Van – bi-monthly to expedite DSS application processing

Weight loss and wellness programs

Seasonal fruit and vegetable program
