HOMELESS SERVICES - SUPPORTIVE HOUSING - FCA

Description

Family & Children's Agency's PILOTS and Next Steps programs provide permanent supportive housing to individuals experiencing homelessness. Supportive Housing provides individuals with a housing subsidy,utilized to securre housing of his/her choice in the community and ongoing in-home Case Management support services.

 

Case Management services are tailored to the individualized needs of the tenant and focus on stablization, maximizing independence and individual goal attainment.  Tenants are linked to mainstream services as well as educational and vocational support.

Monday - Friday

9 A.M. - 5 P.M.

Requirements:

PILOTS: Individual must be homeless either living on the street or in an Emergency Shelter and must have a serious and persistent mental health issue or co-occurring disorder.

 

Next Steps:  Individual must be chronically homeless either homeless for a continuous year or 4 times in the last 3 years and have a serious and persistent mental health issue, co-occurring disorder, or addiction disorder.
